How To Create An Online Profile

After choosing the service, date, and time of your appointment, you arrive at the “We’re Almost Done!!!” page. Select ‘Register/Login’ button.

IF YOU ARE AN EXISTING PATIENT

1. Use the same email address you provided us
2. Hit “Forgot Password?”
3. Re-enter your email address
4. Hit “Reset”
5. Go to your Email with the subject line “MEEVO Notification”
6. Copy the “Reset Code”
7. Return to “Register/Login” Window”
8. Paste the Reset Code
9. Write in Password of your choice!
10. After confirming your new password, hit “Reset Password”
11. Login to your account with your new password

IF YOU ARE NEW TO OUR PRACTICE

1. Push “Sign up now!”
2. Enter your Email Address
3. Create a Password
4. Push “Create Profile”
5. Enter Contact Information!
